This Coaching Agreement ("Agreement") is entered into between Carrie Holladay ("Coach") and client to outline the terms and conditions of the coaching relationship. Please carefully read and understand the following:

 

  1. Coaching Services:
  2. The Coach agrees to provide coaching services to assist the Client in achieving their personal goals.

 

  1. Client's Responsibility:
  2. The Client acknowledges that coaching is a partnership and that they are responsible for their decisions and actions.

 

  1. Nature of the Relationship:
  2. The Coach is a Trauma Trained Somatic Practitioner, not a licensed therapist, and cannot provide any diagnoses. Coaching is not a substitute for professional mental health care or medical treatment.

 

  1. Confidentiality:
  2. The Coach will keep all information shared by the Client confidential unless explicit consent is given to disclose specific information.

 

  1. Duration and Frequency:
  2. The coaching sessions will be conducted virtually via Zoom for a duration of 12 sessions and each session will last 50 minutes.

 

  1. Fees:
  2. The Client agrees to pay the agreed-upon coaching fee of $700/month for 3 months or $350/month for 6 months, depending on the package chosen.

 

  1. Cancellation Policy:
  2. The Client must provide at least 24 hours notice for session cancellations. Failure to do so may result in a full session fee charge. If client does not show up to session, without prior communication to coach, for 15 minutes into the scheduled time, the session is forfeited.

 

  1. Termination:
  2. This agreement is only terminated at the time of purchased package completion.

 

  1. Results Not Guaranteed:
  2. The Coach cannot guarantee specific outcomes or results. Progress is dependent on the Client's commitment and effort.

 

  1. Agreement Acceptance:
  2. By participating in coaching sessions, the Client acknowledges understanding and acceptance of the terms outlined in this Agreement.

 

  1. Governing Law:
  2. This Agreement shall be governed by and construed in accordance with the laws of UT.

 

  1. Amendment:
  2. This Agreement may be amended only in writing and signed by both parties.

By purchasing coaching package, both parties acknowledge and agree to the terms of this Coaching Agreement.

1:1 Biweekly Coaching (6 months)

 

Twelve 50-minute sessions via Zoom biweekly for 6 months

Must be completed within 7 months. Sessions not completed within this grace period will be forfeited without refund.

 

RESCHEDULING:

  •  To reschedule a session with more than 24 hours notice, use the rescheduling link within the Calendly invite to cancel and rebook your appointment. This must be done a minimum of 24 hours before the coaching session is scheduled to begin. If you cancel with less than 24 hours notice or no-show for a session, your session will be forfeited.  Additionally, if you arrive to your session more than 15 minutes after it’s start time, this will be counted as a no-show and client forfeits session.

 

PAYMENT

  •  No refunds are offered for any reason.
  •  Monthly payments will be automatically withdrawn via program’s online site, starting from the date of your purchase and pulled out on the same day each month.
  •  If a payment for any reason fails to collect, you will not be able to schedule or keep any future sessions until payment is completed.
  •  If payment is not provided within 90-120 days, the client’s past-due balance will be deferred to a debt collection agency.
